How to apply for Chiayi University production and sales resume verification?!
1. Please fill in - Chiayi University Agricultural Products Production and Sales Resume Verification Application Form
2. A photocopy of the front and back of the ID card of the person in charge of the farm/farm or a photocopy of the company’s for-profit business registration
3.Basic information about the breeding farm/farm：
－Copy of cadastral data of the production area or aquaculture fishery registration certificate
－If you are a lessee, you must provide a copy of the lease certificate or the owner’s consent to use.
－Breeding farm/farm layout diagram (breeding pond needs to be marked)
－Relative location distribution map, nearby transportation route map
4. Production and sales resume standard operating procedure TGAP
5. Please go to the "Aquatic Products Production and Sales Resume Information Network" to apply for an organization account
6. Complete production and sales records for the three months as of the date of application must be attached.
7. Breeding farm/farm self-assessment - self-assessment form (recorded at least once a month)
8. Independent inspection report within one year (items that must comply with TGAP regulations)
